In consideration of and as a condition of the below listed player’s (the “Player”) acceptance and
participation in the Unified Sports League Birmingham/The Kind Bison, Inc. (the “Unified/KB”) basketball
league to be played at the Levite Jewish Community Center in Birmingham (the “JCC”) and recognizing
the possibility of physical injury associated with basketball, I hereby for the Player and myself listed
below, our heirs, executors and administrators, waive and release, discharge and/or otherwise indemnify
the JCC and its associated personnel against any claim by or on behalf of myself or the Player resulting
from the Player’s participation at the JCC that is now existing or hereafter may exist for damage or injury
to the Player, or to any person or property, resulting from the negligence or other acts of any employees
or volunteers in connection with the Player’s participation at said facility.
I further agree to indemnify the JCC and Unified/KB (including their officers and associated personnel)
free and harmless from any loss, liability, damage, cost or expense which they may incur as a result of
any injury and/or property damage that the Player or I may cause or sustain while participating in
Unified/KB at the JCC. I further agree that this waiver, release and assumption of risks shall be binding on
the heirs and assigns of the undersigned and the Player. I understand that medical insurance coverage is
necessary and required for the Player’s participation in Unified/KB at the JCC, and that adequate
coverage is the responsibility of the parent or guardian of the Player.
Recognizing the possibility of physical injury associated with basketball and in consideration for
Unified/KB accepting registrant for its basketball league and events, I hereby release, discharge, and/or
otherwise indemnify Unified/KB, its affiliated organizations and sponsors, their officers, volunteers, and
associated personnel, including the owners of the facilities utilized, against any claim by or on behalf of
my child's participation in any and all Unified/KB events and/or being transported to and from the same.
My child has received a physical examination by a physician and has been found physically capable of
participating in the sport of basketball and by extension this league. I also assume the financial
responsibility for any medical treatment for my child.
Further, I understand that there may be individual and team photographs taken of the players at
Unified/KB and its events. These pictures may also be displayed in local newspapers, newsletters, social
media, and the Unified/KB website. I give Unified/KB permission to photograph my son/daughter
individually and as part of a team knowing that this picture may be used for the purposes described
above.
